Please complete this form so we may arrange the return of your Upfront Edge device.
Upon completion of your Hardware Upgrade, your account manager will send a return kit and pre-paid waybill to you with your order to return the Upfront Edge device.
I acknowledge I will be charged the value of my Upfront Edge discount (remaining balance) if I fail to return my device within 30 days and/or any of the information provided is found incorrect.
